Authorization Form

The Authorization Form generated by FormFox Order contains many of the simplifications that make the system so wonderful and easy to use. Once the test has been ordered online, this Authorization Form can be printed and handed to the employee or their manager or can be emailed directly to the employee or manager for them to print.

The authorization barcode is scanned by the collection site and contains all the information found on this page.

Donor / Test Info

Information about the donor, test dates, reason for the test and the services to be performed.

Collection Site Info

Information about the selected FormFox-enabled site including the address and any special notes such as parking instructions.

Map To Site

A google map is generated to assist in locating the designated collection facility.

The form contains a reminder of the scheduled time and date along with a map and the address of the collection facility. The Authorization Barcode is scanned upon arrival at the site and all test / donor information is received digitally (leaving no chance for human entry error).
